Check in goes faster when forms are filled out in advance.
Blank Waivers for Mass Printing:
Please be sure to print clearly, if we can't read it you may be asked to fix the un-readable areas. The blank waiver will print 2 pages one of which will be blank, do a print preview and change setting when nessisary.
Fill out on-line, when done print and bring it to the field. If printing from firefox, you may need to adjust the scale of the printing to 60% other wise the letter are cut off. (I spent a full day trying to fix the problem, but it's not working, even thought the other forms seem to work just fine.)
